Earlier reports had put the death toll at six.

The number of injuries stood at five, after one person was treated and released from a local hospital, CNN reported, citing a nurse at the hospital.

The shooter, who was among those injured and being treated at the hospital, was not a patient, CNN and local broadcasters reported, citing local officials.

The shooter, a male, forced his way into the facility in the morning and opened fire, Carthage Police Chief Chris McKenzie said.

The Pinelake Health and Rehab houses about 90 people with special needs, many suffering from Alzheimer’s. No motives for the attack were known. A press conference was planned for 2100 GMT.

Carthage is about 100 km from Raleigh, the capital of North Carolina.
