Lucknow: The Uttar Pradesh state Haj Committee has appealed to the aspirants for Haj-2011 to enclose two copies of their international passport while submitting their Haj application forms. In case of non availability of international passport, the receipt for having applied for passport should be submitted along with the form. The Haj forms are available at committee’s office from March 16 and the last date for their submission of Haj forms is April 30.
Officials at the state Haj committee had informed that nine counters for issuance and depositing the forms have been set up at the committee office. A helpline number 9235610680 and website http://www.uphajcommittee.com/ too has been updated and made available for any query. The devotees are also requested to write their mobile number on the application forms. The forms will be accepted in two copies (both original) along with all the necessary documents including the copy of international passport.
